Setting up virtual rehabilitation ward

Virtual Rehabilitation Ward (VRW) was established in January 2022 as part of a hospital avoidance strategy in Southern Adelaide Local Health Network. This allows the patients to undergo intensive rehabilitation in their home and be monitored medically while at home. The workshop’s aim is to provide a template into setting up a virtual rehabilitation ward which is different from the usual home rehabilitation program as we focused on delivering a virtual first approach.

VRW’s cohort of patients are also those who requires increased medical and nursing input as they are medically complex who requires ongoing monitoring of their vital signs to ensure that they can continue to engage in rehabilitation program at home with aim to minimise their presentation back to hospital.
